Job Summary
This role is responsible for developing, implementing, and managing initiatives that support value-based care models and population health strategies. This role collaborates cross-functionally with clinical, financial, and operational teams to ensure performance under value-based contracts, improve patient outcomes, reduce costs, and support the transition from fee-for-service to value-based reimbursement structures.
Roles and Responsibilities
* Leads system-wide efforts to improve patient outcomes, reduce variation, and achieve clinical best practices.
* Develops and executes strategies to succeed in value-based payment models including ACOs, bundled payments, shared savings, and risk-based contracts.
* Monitors performance metrics (quality, utilization, cost) across VBC arrangements and identify opportunities for improvement.
* Leads Clinical Quality team on improvements and focused driven outcomes.
* Collaborates with senior clinical leadership on the development of Clinical Pillar Goals.
* Assists with development and implementation of company policies and procedures.
* Provides support, resources, knowledge, advice and/or counsel to leadership at campus and clinical teams in accordance with current rules, regulations, and guidelines that govern the campus.
* Develops relationships with regulatory agencies and professional organizations.
* Keeps current with CMS guidelines and regulations and current standards of practice.
* Maintains awareness of laws and regulations, keeping abreast of current changes that may affect healthcare systems. Utilizes personal initiative, seminars, training programs, peer contact, and various industry information sources. (e.g., internet sites, newsletters, etc.)
* Other duties as assigned.
Qualifications
Education: Bachelor Degree
Experience: 12-15 years
Licenses and Certifications
RN
RAC-CT or strong experience, preferred.
Physical Requirements
Sitting, standing, bending, reaching, stretching, stooping, walking, and moving intermittently during working hours. Must be able to lift at least 50lbs. Must be able to maintain verbal and written communication with co-workers, supervisors, residents, family members, visitors, vendors, and all business associates outside of the health campus.
