Novant Health is seeking an experienced Therapist III that is able to adapt to each unique client, as well as display proficient knowledge and experience in implementing and coordinating treatment plans while considering various cultures and age groups.
The Therapist III is responsible for making treatment and level of care decisions independently. They collaborate as needed with their physician providers. As an autonomous provider in the outpatient setting, the therapist is responsible to manage their own schedule, refer patients as needed to a higher level of care or request consultations. They are individually credentialed with payers to bill for services. A referral is not required to initiate treatment.
The Therapist provides individual, family, group therapy and collaborative care as appropriate in the outpatient setting. They complete patient assessments, plan for therapeutic interventions and appropriate follow up care, implementation treatment plans, coordinate the treatment process.
Also demonstrates the knowledge and skills necessary to provide services appropriate to the age and cultural diversity of the patients served. Schedule:

Responsibilities:

* Palliative Care Team participation:
* PCSW will act as part of the larger Palliative Care Team, communicating clearly and effectively with other team members to provide quality patient and family care, meet individual patient and family members care needs, and achieve appropriate palliative care plans.
* PCSW will participate in and contribute to weekly team rounds.
* PCSW will attend and contribute to team meetings.
* Psychosocial assessment and support:
* Assessment: Palliative Care Social Worker (PCSW) will perform a comprehensive patient and family biopsychosocial-spiritual assessment.
* Counseling PCSW will engage with patient and family to facilitate trust, rapport, and communication. Provide brief supportive counseling, crisis intervention, or intensive therapeutic supports as needed to the patient, family group or individual family members.
* Bereavement support Anticipation and assessment of bereavement support needs during the patient's hospital stay; Discussion of complicated bereavement risk for surviving family members and providing referrals or resources as indicated
* Advance care planning, will act as liaison to promote discussion between the patient, family, and medical team, in order to facilitate understanding of the patient's medical condition and treatment options, and to establish clear, patient-centered goals of care and care plans.
* Education, PCSW will provide education and guidance to the patient, family and care team members pertaining to palliative and end-of-life care.
* Referral, PCSW will refer the patient and family to appropriate supports and resources.
* Advocacy, PCSW will play a unique role in advocating for the patient and family to ensure respect for patient autonomy and to maximize the safety of the patient's care plan.
* Disposition, PCSW will make recommendations for care plan and disposition, and will remain available to treatment team, patient and family for ongoing consultation.
* Documentation, PCSW will document on all patients seen inpatient and outpatient. PCSW documentation will serve to enhance communication and model interdisciplinary work and palliative language in the medical record.
Qualifications:
* Education: Master's Degree in Psychology, Social Work, Counseling or related field required.
* Experience: Minimum of three years' experience required.
* Licensure/Certification/Registration: LPC, LCSW, Clinical Psychologist or PMHCNS required.
* Additional Skills Required: Good working knowledge of computers. Ability to care for Behavioral Health Age Specific populations. Non-Violent Crisis Intervention certification.
