Responsibilities:

Summary

We are seeking a team of Temporary Patient Transition Specialists to support a key summer project resulting from a recent acquisition. This role will involve making outbound calls to patients who are transitioning to our services from a previous Durable Medical Equipment (DME) provider. Your goal will be to facilitate a smooth transition by collecting basic demographic and clinical information, documenting it accurately, and helping to ensure all necessary documentation is in place.

* This is a temporary remote position expected to run during the summer months
* Part-time hours (approx. 30/week) with scheduling flexibility within the hours of 10 AM to 7 PM ET
* All necessary training and system access will be provided

Essential Duties and Responsibilities

* Contact patients to gather demographic and clinical information using a standardized form
* Upload completed information into our internal eIntake system
* Communicate with physician offices to obtain prescriptions and required documentation for billing
* Ensure that patients meet the qualifications for equipment prior to initiating service transition
* Report status updates to the Field Operations Manager regarding field assignments and documentation progress
* Support the implementation of best practices for operational efficiency during the transition
* Perform additional related tasks as needed within the scope of the transition project

Qualifications:

Employment is contingent on

* Background investigation (company-wide)
* Drug screen (when applicable for the position)
* Valid driver's license in state of residence with a clean driving record (when applicable for the position)

Education and/or Experience

* High school diploma or GED equivalent, required
* Experience in healthcare, DME, or customer service is a plus
* Strong customer service background

Skills, Knowledge and Abilities

* Excellent communication skills, both written and verbal
* Strong active listening and interpersonal skills
* High attention to detail and accuracy in data entry
* Self-motivated with the ability to work independently
* Compassionate and empathetic approach to patient interactions
* Proficient in computer systems and software; ability to learn new platforms quickly
* Experience in healthcare, DME, or customer service is a plus

Physical Demands

* Requires sitting, walking, standing, talking or listening
* Requires close vision to small print on computer and or paperwork; utilize computer for extensive hours

* Email transmission and communication
* Internet navigation and research
* Microsoft applications including but not limited to Word, Excel, Power-Point, etc.
* Office equipment; fax machine, copier, printer, phone and computer
* Rotech systems, including eIntake
