What You Can Expect:
* Provides day-to-day oversight of designated teams across multiple sites.
* Provides leadership, guidance, counseling and continuing education to staff. Will select, develop, coach, mentor, and assess performance of staff.
* Assists with coordination of schedules to ensure adequate staffing levels.
* Resource and operational expert who collaborates with and supports the area leadership in problem-solving and utilization of resources.
* Coordinate quality initiatives and process improvements for designated areas. Tracks and analyzes performance metrics and reports, as requested.
Qualifications:
* Education: Associate of Nursing, Bachelors of Nursing
* Experience: 5 years' experience (ASN) or 3 years' experience (BSN)
* Licenses/Certifications: Must hold a license in the state of practice in one of the following: RN in Idaho
Responsibilities:
The Supervisor of Case Management and Social Services provides supervision and direction to staff across care/case management and social/behavioral services within designated facilities. Provides guidance on best practices in care coordination, supporting team members in connecting patients with appropriate internal and community resources, and ensuring effective utilization management.
-Provides day-to-day oversight of designated teams.
-Provides leadership, guidance, counseling and continuing education to staff. Will select, develop, coach, mentor, and assess performance of staff. Participate in professional development and maintain current knowledge of healthcare management and case management standards.
-Assists with coordination of schedules to ensure adequate staffing levels. Supervises case management staff and suggests reassignment of workload to maximize performance as needed. Ensures the departments and hospitals operate efficiently with respect to the staffing ratios and coverage for planned and unplanned time off.
* Serves as a resource and operational expert, collaborating with area leadership to support problem-solving and effective resource utilization. Assists in identifying and implementing process improvement initiatives within daily workflows to enhance operational efficiency.
-Coordinates quality initiatives and process improvements for designated areas. Supports the implementation, communication, and compliance of new policies and procedures, while ensuring ongoing adherence to existing ones. Tracks and analyzes performance metrics and reports, as needed.
* Serves as a subject matter expert to support staff in resolving complex issues within the area of oversight. Acts as a mentor and trusted resource, fostering staff development and promoting best practices.
-Fosters a collaborative relationship with interdisciplinary teams.
-Assumes responsibility for the team in the absence of the Manager of Case Management and Social Services, ensuring continuity of operations and support.
-Perform other duties and responsibilities as assigned.
Qualifications:
* Education: Associate of Nursing, Bachelors of Nursing, or Master's of Social Work
* Experience: 5 years' experience (ASN) or 3 years' experience (BSN/MSW)
* Licenses/Certifications: Must hold a license in the state of practice in one of the following: RN, Licensed Clinical Social Worker (LCSW)
