We are currently hiring for a Full Time Stroke Nurse Navigator! What You Can Expect: The Nurse Navigator educates patients and caregivers, coordinates post stroke care between patient and providers, offers follow up and ongoing stroke survivor support, and assists with access to community resources. The Nurse Navigator will also follow up with patients post discharge to ensure patients have met with their PCP, filled their prescriptions, completed their outpatient diagnostics, and followed all other post discharge stroke clinical practice guidelines. Schedule: Monday-Friday 8:00am-5:00pm This role will work out of Boise Medical Center. Qualifications: * Education: Nursing Degree * Experience: 2 years' of RN experience * Licenses/Certifications: Current RN License in the State of Practice Responsibilities: The Nurse Navigator offers medical guidance, acts as a support group, and guides patients and their families through the treatment process. * Serves as a single point of contact for patients undergoing treatment,. * Serves as a liaison between the patient and their family and the patient's network of care providers. * Educates patient on diagnosis, treatment options, and prognosis. * Assists the patient in scheduling appointments, completing paperwork, or securing referrals. * Facilitates short term and/or long term interventions. * Possesses comprehensive knowledge of subject matter. * Handles complex issues and problems, and refers only the most complex issues to higher-level staff. * Other duties and responsibilities as assigned. Qualifications: * Education: Bachelors of Nursing Degree * Experience: 2 years's of RN experience * Licenses/Certifications: Current RN License in the State of Practice AND Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross.
