Responsibilities:

The Home Health Supervisor develops a home care plan of instructing patient, family/caregivers and other nurses to implement the plan for therapeutic treatment and rehabilitation of the speech impaired. Responsible for maintaining confidentiality per Mercy Medical Center's Home Health's administrative policy.

Patient Care:

* Perform patient assessment, including giving or receiving report, obtaining the patient history, conducting a physical assessment, establishing priorities for care. Assess patient's learning needs, readiness for discharge and ability to manage illness after discharge.
* Completes documentation timely and submits within 24 hours of visit. Contact the Supervisor and update him/her on the patient's progress. Submits schedule weekly and notifies Scheduler of changes
as they occur.
* Maintains productivity standards as set by the department.
* Receive and administer physician orders in a safe and timely manner. Uses hospital/agency approved processes for telephone and/or verbal orders.

Develop/Implement Interdisciplinary Care Plan:

* Develop/implement care plan and/or discharge plan for each patient, including interpreting care paths, physician orders, progress notes. Provide direct patient care, as required. Perform point-of-care testing where applicable. Contact a physician with any problems or concerns, as necessary. Report changes in the patient's condition to the physician. Maintain a safe, clean and therapeutic environment for patients
* EVALUATE/DOCUMENT INTERDISCIPLINARY CARE PLAN: Prepare or assign all required documentation to meet legal requirements and hospital policies and procedures. Maintain appropriate communication with other staff members, patients, and family members, including answering questions, discussing patient care, and providing emotional support to patients and family.

Qualifications:

Minimum:

* One year of clinical experience with direct patient contact
* CA Speech Therapist License
* American Heart Association Basic Life Support (BLS)
* Valid CA Driver's License
* Proof of car insurance upon hire
* Ability to read, analyze, and interpret hospital/agency policies and procedures, professional journals, technical procedures, or governmental regulations.
* Ability to write reports and procedures and document in the medical record.
* Ability to effectively present information and respond to questions from co-workers, managers, patients, physicians, family members and the general public.
* Must be able to read, write, and speak English proficiently.
