Responsible for coordinating and implementing post-discharge plans in coordination with the Case Managers through the use of Extended Care Information Network (ECIN). They are also responsible for assisting with advocacy and referrals to other community resources. Qualifications: In addition to bringing humankindness to the workplace each day, qualified candidates will need the following: Master Degree Social Work (Required) Knowledge of community resources used for discharge planning, hospital operations, excellent communication/presentation skills, knowledge of third party payment systems, Medicare/Medicaid programs. Maintains current knowledge base of community services through continuing education. Ability to multi-task, set priorities and maintain organization. Computer skills. Licensed Clinical Social Worker (Required) Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly This is an inpatient Social Worker role that requires a LCSW. The schedule for this role is 8:00 am to 4:30 pm. This role will flex as needed with accordance to low census as needed.
