Responsibilities:

Social Worker - St. Bernardine Medical Center

St. Bernardine Medical Center is seeking a compassionate and skilled Social Worker to join our multidisciplinary care team. In this role, you will provide critical social work assessments, interventions, and discharge planning for patients in inpatient, outpatient, and emergency department settings. Your efforts will directly support our commitment to patient- and family-centered care, with a strong emphasis on dignity, ethics, and holistic healing.

Key Responsibilities

* Conduct comprehensive psychosocial assessments and develop treatment plans addressing social, emotional, financial, and resource-related challenges.
* Collaborate with physicians, nurses, and case managers to implement effective, patient-centered care plans.
* Provide crisis intervention, short-term counseling, and advocacy for patients and families.
* Coordinate with community agencies, financial assistance programs, and post-acute care services.
* Assist in complex discharge planning, arrange care transitions, and engage community resources for high-risk patients.
* Participate in hospital committees and deliver staff education and in-service training as needed.

Qualifications:

* At least one year of clinical and/or medical social work experience required
* Masters Degree from a School of Social work accredited by the Council of Social Work Education Knowledge of contemporary behavioral and systems theories relevant to health care; end-of-life dynamics and interventions; grief and bereavement counseling; substance abuse identification and interventions; support for victims of abuse neglect or violence; community or financial resources for underserved or vulnerable populations.
* SSDI SDI and other governmental/county requirements/programs.
* Thorough comprehension of OBRA and Title 22 is required.
