Shift:  Monday-Friday; 8:00 AM - 4:30 PM; occasional on-call + holiday on call rotation

As a key member of our interdisciplinary care team, you'll play a vital role in supporting patients through every step of their care journey, from hospitalization through discharge and beyond. You'll work closely with nurses, physicians, therapists, and community partners to ensure patients receive the comprehensive support they need, both physically and emotionally.

Responsibilities:

* Take charge of discharge coordination to ensure smooth, timely transitions of care that support both excellent patient outcomes and efficient patient flow within the hospital.

* Evaluate each patient's clinical condition and psychosocial needs, identifying risk factors and designing a plan of care.

* Pinpoint essential interventions and work closely with physicians and primary nurses to tailor care plans to the patient's specific situation and goals.

* Build trust and collaboration by working directly with patients and their loved ones, ensuring their voices are heard and their needs are met.

* Play a central role in care coordination rounds, complex care discussions, and unit-based team meetings, promoting clear communication and unified planning across the care team.

Qualifications:

Education:

* Master's Degree in Social Work required.

License(s)/Certification(s):

* Social Work license in Iowa and Illinois required.  License in second state must be obtained within 30 days of hire.
* CPR certiciation upon hire.
* Valid driver's license when driving any vehicle for work-related reasons.

Experience:

* 1-2 years post graduate experience.

*Specific offers are determined by various factors, such as experience, skills, internal equity, and other business needs.  The salary range listed does not include other forms of compensation which may include bonuses/incentive, differential pay, or other forms of compensation or benefits that may be applicable to this role.
