Responsibilities:
* Work with Registered Nurse Manager to provide timely and efficient identification of patients who need discharge planning.
* Provide assessments of psychosocial impact of illness.
* Provide direct services including discharge planning for patients needing community resources prior to discharge from the acute care setting.
* Experience in a medical setting is preferred.
Qualifications:
* A current unencumbered Social Worker license to practice in the State of Arkansas is required,
* BLS within 30 days
* Must be able to communicate effectively, both written and oral.
* Must possess organizational skills, able to prioritize work.
* Must possess demonstrated training, knowledge, and skills necessary/appropriate to the adult, and/or geriatric age groups
