What You Can Expect:
* 12 Hour Shifts, Thursday, Friday and every other weekend
* Shift Differentials for weekends and nights
* 12:00-Midnight

Qualifications:
* Education: Masters degree.
* Licenses/Certifications: Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in Idaho
Responsibilities:
The Case Manager for Social Work works with physicians and multidisciplinary team members to develop a plan of care for assigned patients.
* Works as consultant, educator, or community liaison to facilitate collaborative coordination of patient care.
* Uses assessment skills and risk assessment tools to identify patients with actual or potential health care needs that would require transitions of care coordination.
* Collaborates with patient/family and interdisciplinary team in establishing mutual goals based on the patient's needs or problems.
* Coordinates care across the continuum (inpatient/outpatient/community) to assure appropriate utilization of clinical and community resources.
* Applies specialized expertise to solve problems, generate ideas, plan care, and teach.
* Evaluates patient's progress towards identified goals.
* Assists the care team by identifying potential barriers that impedes access to care. Implementing plan to address barriers.
* Coordination of primary care and sub-specialty providers.
* Serves as subject matter expert within specialty area.
* Perform other duties and responsibilities as assigned.
Qualifications:
* Education: Masters Degree in Social Work
* Experience: 0 years experience
* Licenses/Certifications: Licensed Clinical Social Worker (LCSW) in the state of practice.
