Overview: As the Social Services Director you will identify and provide for each resident's social, emotional and psychological needs, and the continuing development of the resident's full potential during his/her stay at the facility and to assist in the planning for his/her discharge. Responsibilities: * Assist residents and their families with social, emotional, and family issues. * Provide timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies. * Participate in resident care planning. * Participate in the development of Discharge planning and prepare resident for the transition. * Serve as a general source of information for residents and their families. Qualifications: * Master's Degree in Social Work or Psychology. * 3+ Years Experience working in Social Services, directly with individuals, preferably in a healthcare setting. * High level of professionalism and ability to maintain confidentiality.
