				Overview:

As a  Social Service Staff you will assist the Social Services Director/Coordinator in identifying and providing for each resident's social, emotional and psychological needs, and the continuing development of the resident's full potential during his/her stay at the facility and to assist in the planning for his/her discharge.

Responsibilities:

* Assist residents and their families with social, emotional, and family issues. 
* Assist in prividing timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies. 
* Participate in resident care planning and discharge planning.  
* Serve as a general source of information for residents and their families 

Qualifications:

* At least one (1) year of experience working in social work, preferably in healthcare setting. 
* High level of professionalism and ability to maintain confidentiality. 
* Effective Verbal and Written communication skills.
