Overview:
The Site Coordinator (SC) will support an innovative care delivery model, Rural Home Hospital. The program offers a subset of patients an alternative to standard hospital admission, in which patients can receive highly specialized acute-care services in their home. The SC will follow patients via medical record, assist with patient enrollment, and engage with patients to provide consent information. The role also has follow-up functions of both hospitalized and post-discharge patients via telephone. For example, the SC calls patients to collect data for post-discharge surveys.
Responsibilities:
* Involved with patient recruitment, provide consent information to eligible patients.
* Assists in preparing equipment and supplies for patients.
* Arranges transportation for patients as needed.
* Administers surveys and interviews to consented patients.
* Administers follow up surveys via telephone.
* Completes quality and safety reporting as required.
* Performs data abstraction from various sources, including patient medical records, hospital clinical information systems, and surveys.
* Assists with qualitative analysis.
* Utilizes database systems to enable and track data collection efforts, including the development of case report forms and descriptive data analyses.
* Assists clinical team with care coordination, such as requesting outside medical records, scheduling follow up appointments for patients, processing prior authorizations, and ordering durable medical equipment.
* Maintains inventory and orders supplies.
* Cleans and maintains equipment.
* Assists with photocopying, sending mail, faxing, documenting project work, and other administrative duties as required.
Qualifications:
Minimum Education: ADN-BSN-MSN or master's degree in healthcare discipline preferred. Graduate of an accredited baccalaureate program of nursing and active RN license required.
Minimum Work Experience:
* Minimum of two (2) years ICU and/or acute care nursing.
* Minimum of three (3) years advanced knowledge of stroke nursing experience.
* One (1) to three (3) years of management experience preferred.
* Evidence of clinical expertise in the practice of stroke/critical care.
Required Licenses/Certifications:Registered Nurse License, TNCC, RTTDC NIHSS Certification obtained within 90 days of hire date.
Required Skills, Knowledge, and Abilities: Must have the ability to provide on-site coverage seven days a week with hours ranging from 8a to 7p, including weekends and holidays.
Associates Degree required.
