Overview: Responsible for providing pre-admission intake assessments for acute inpatient rehabilitation services. Serves as one of the primary facilitators for patient referrals from internal and external referral sources to IRU. In this role the rehabilitation referral liaison will be expected to interact with discharge planners, insurance carriers, physicians, patients, family members and other related parties who may be involved with or inquiring about the rehabilitation programs. Assures the pre-admission intake assessments meet admission criteria by St. Luke's, CMS regulations, and C.A.R.F. standards. In addition, this associate is expected to adhere to and exemplify the UnityPoint Focus Values and the PMR associate-patient relationship policy. Responsibilities: Pre-admission Referrals/assessments * Acknowledges all referrals to IRU and communicates to team via the consult pending list. * Is knowledgeable of post-acute levels of care and their admission criteria, i.e. IRU, SNF, LTACH. * Assures IRU admission criteria in accordance to CMS and CARF regulations are met. * Conducts pre-admission referral assessment and completes the required documentation in E.H.R. * Communicates the pre-admission assessment to rehab physician for further review and admission decision. * Meets referred patient/family to review the IRU program/services and identify goals and discharge plan prior to IRU admission. * Provides tours of the IRU to families and other stake holders. Marketing/Referral Relationship development/volume growth * Foster's excellent communication between referral sources, physicians, insurance companies, and the interdisciplinary IRU team. * Participates in performance improvement opportunities related to IRU referral process. * Assures monthly/quarterly referral tracking data is complete and accurate. Insurance benefit verification and Preauthorizations * Verifies that referred patient has IRU benefits and performs all required pre-admission authorizations prior to IRU admission. Qualifications: * Graduate of either an accredited school of nursing-B.S.N. degree preferred or a graduate of a Physical or Occupational Therapy Program. * Licensed RN, P.T., or O.T. in the state of Iowa. * Minimum of 2 years in Rehabilitation services * Computer experience required. Must be able to use Word.
