What you can expect as a Speech Language Pathologist- Rehabilitation Home Care Coordinator:
* Schedule: Monday-Friday 8:30AM-5PM
* Manageable case-loads (Daily visit requirements below state and national average)
* Paid for drive time
* Mileage reimbursement (Geographical Area: Meridian team serves the following counties in Idaho: Ada, Boise, Canyon, Elmore, Gem, Owyhee, Payette, and Washington counties)
Qualifications:
* Education:
* D Degree from an accredited speech therapy program
* Experience:
* 2 years relevant experience
* Licenses/Certifications:
* Valid license to practice speech therapy in state of practice.
* Valid Driver's License and Auto Insurance.
* Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross
* Preferred Qualifications:
* Home Health experience
Responsibilities:
Speech Language Pathologist (SLPs) are health care professionals who develop, improve, sustain, or restore independence in activities required in daily life to any person who has an injury, illness, disability or psychological dysfunction. Speech Language Pathologist responsibilities involve advanced level duties. Serves as role model for the care of patients with conditions common to their area of expertise.
* Perform initial and ongoing comprehensive evaluation including, but not limited to, collecting subjective and objective data, analysis of data, and creation of treatment plan.
* Administers interventions consistent with treatment plan based on evidence based best practice, provider protocol and governing standards.
* Follows policies and procedures in accordance with all regulatory agency requirements.
* Document care based on standards of practice, patient acuity and/or clinical priorities.
* Maintain professional relationships and convey relevant information to other members of the healthcare team within facility and any applicable referral agencies.
* Maintain patient confidence and protects organization operations by keeping information confidential.
* Independently manages the care of patients with conditions common to the unit/area of assignment. Exercises initiative and independent judgment. Manages caseload and maintains productivity standards for unit consistently.
* Serve as clinical instructor to students, as appropriate.
* Serve as a clinical mentor, as appropriate.
* Direct the work of aides and assistants, as appropriate.
* May provide in-service training in areas of recognized expertise at the local, regional, and state level and mentor students during fieldwork experience.
* Provide staff and/or community education in area of clinical expertise.
* Identifies opportunities for program improvement and leads team for implementing change, seeking guidance, as appropriate.
* Maintain evidence-based protocols/pathways and clinical practice in area of clinical expertise
* Assist in implementation of, and adherence to clinical practice and/or pathways.
* Demonstrates ability to perform additional job duties that may include mentorship/teaching for other staff members and students, protocol and program development, and research.
* Demonstrate additional education and/or certification in area of clinical expertise.
* Perform other duties and responsibilities as assigned.
Qualifications:
* Education: Degree from an accredited speech therapy program
* Experience: 3 years relevant experience
* Licenses/Certifications: Valid license to practice speech therapy in state of practice. Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross. ASHA Certificate of Clinical Competence
