What You Can Expect From This Role: * Schedule: Full Time, 8 hour weekday shifts * Benefits: Full Benefits with hospital contributions * Training: Paid onboarding and training Qualifications: * Education: Bachelors degree * Licenses/Certifications: Licensed Social Worker (LSW) in Idaho For more information, please contact Julie at leguinej@slhs.org Responsibilities: Under supervision, the Licensed Social Worker (LSW) provides psychosocial assessment and intervention to patients and families across a continuum of care. * Interviews patients and relatives to obtain social history relevant to medical problems and planning. * Assists patients with environmental difficulties that interfere with obtaining maximum benefits from medical care. * Serves as a member of a multi-disciplinary team and is a liaison with community resources. * Serves as liaison between medical and nursing staffs, patients, relatives and appropriate outside agencies. * Interprets and assists in resolving social problems that relate to the medical condition and/or hospitalization. * Documents assessments, plans and outcomes. * Other duties and responsibilities as assigned. Qualifications: * Education: Bachelors degree * Experience: 0 years experience * Licenses/Certifications: Licensed Social Worker (LSW) in state(s) of practice.
