Position Overview
As a Home Health RN, you will provide in-home care for patients who have been recently diagnosed, discharged from the ER, or require ongoing respiratory treatment, wound care, or medication management. You'll conduct home visits to monitor patients' progress with their care plans, educate them on their conditions, and assess both their physical and emotional well-being. When patients are ready for discharge, you will coordinate resources and educate both the patient and their family, ensuring they have everything they need to continue their recovery at home.
What You Can Expect from This Role
* Flex Schedule: Required to work one shift per pay period. Days of the week will vary Mon-Fri 8:30 AM to 5:00 PM, may include evening on-call and weekend shifts
* Low Visit Requirements: Daily visit expectations are below the state and national average
* Compensation: Paid hourly which includes drive time and mileage reimbursement for traveling within McCall and its surrounding regions
* Additional Pay: On-call and applicable differential pay

Qualifications
* Education: Nursing Degree (ASN or BSN)
* Experience: At least 1 year of RN experience; Home Health or Acute Care experience is a plus
* Licenses/Certifications: Current RN License in the state of Idaho; Basic Life Support (BLS) certification
* Other Requirements: Valid driver's license and proof of auto insurance

Responsibilities:
* Works as clinician, consultant, educator, and researcher to facilitate collaborative management of patient care.
* Works closely with physicians, nurses, social workers and a wide range of medical and non-medical professionals.
* Ensures patient is progressing towards desired outcomes by monitoring care through assessment and/or patient records.
* Determines need for therapeutic medical, psycho-social, and psychiatric evaluations. Reviews therapist evaluations, treatment, objectives, and plans.
* Develops patient care plans, evaluates and monitors progress, responds to patient and family needs. Serves as advocate when problems and issues arise.
* Coordinates discharge and post-discharge requirements; orientates and trains family members; provides resources.
* Performs therapeutic nursing interventions under the direction of a licensed physician.
* Provides ongoing patient education on diagnosis, treatments, prognosis and medications.
* Perform other duties and responsibilities as assigned.
Qualifications:
* Education: Bachelor's of Nursing Degree
* Experience: 2 years of RN experience
* Licenses/Certifications: Current RN License in the State of Practice
* Valid Driver's License and Auto Insurance if work related travel is required for position
