Who We Are:
Abe Brown Ministries (ABM) is a non-profit, Faith-based organization that has been in operation for more than 45 years. The inspHire Program is responsible for providing job placement and other support services for individuals with a lived incarceration experience, regardless of their religious preference, who are being released to Hillsborough, Pinellas, Polk and Pasco counties or an ex-offender referred to Abe Brown Ministries.
 
We are looking for Re-Entry inspHire Program Case Manager to provide services to clients who are admitted into ABM's inspHire Program. The Case Manager plans and organizes collaborative services and develops and administers a Plan of Care for each client, to effectively assist the client with re-entering into society. The Case Manager achieves consistent interactions with clients which include the distribution of services, phone calls and one-to-one sessions for up to a year.

 Job Duties:
* Network: Partners with Florida Department of Corrections (FDC) to recruit potential clients from correctional facilities, transitional housing facilities, Halfway Houses, and Work Release Facilities. Schedules events or attends community events to raise program awareness and gauge community interest. Weekly, visit FDC supervision offices and local re-entry correctional institutes to present ABM's inspHire Program and collect information on potential clients.

* Casework: The Case Manager uses a variety of techniques to determine clients' needs, subsequently completing individual Plan of Care assessments and interviews. Based on the outcomes of implemented and monitored treatment plans, makes independent professional recommendations that have a distinct effect on the client's reentry opportunities. Maintains or exceeds the ABM established standards for program performance relative to each client's growth and development through program participation. Resourcefully manages the client's professional and personal success.

* Community Partnership: Communicates, connects with, and collaborates with community agencies to offer program clients a wide range of resources. Interprets available resources and skillfully conveys information to clients, ensuring they receive, embrace, and begin their best program experience. Follow-up with clients and agencies as appropriate to document use/referral success.

* Administrative: Aligned with ABM Guidelines to use professional verbiage, the Case Manager meticulously enters all client interactions and transactions into the ABM Efforts-to-Outcomes (ETO) data system.

Qualifications:

* Bachelor's degree, master's degree preferred.
* At least two years human services experience.
* Comfort in using computers and learning new software.
* Experience working with the underserved and persons in crisis.
* Strong documentation skills
* Excellent written and verbal communication skills, ability to establish rapport.
* Ability to work in a variety of settings with culturally diverse families and communities with the ability to culturally sensitive and appropriate.
* Ability to motivate others towards achieving goals.
* Must be able to maintain confidentiality involving both clients and fellow employees.
* Ability to legally operate a motor vehicle.
* Maintain professional appearance and comply with company dress code.
