The RN Surgical Liaison utilizes effective communication and collaboration skills to ensure coordination of care with physicians, nursing staff, other disciplines/departments, and patients to facilitate positive patient outcomes. The RN Surgical Liaison plans effectively in order to meet patient needs. Responsibilities: A day in the life of a RN Surgical Liaison at Hackensack Meridian Health includes: * Facilitates the entry of patients into the Surgical Gynecologic Oncology Division * Provides the initial assessment of patient by obtaining the medical and surgical history, allergies, medications, and family history. * Obtains prior imaging studies. * Communicates the findings to the MD prior to the MD visit. * Provides patient education across the continuum of care to obtain optimum surgical results. * Patient education done pre and post-operatively. * RN is responsible to inform the patient what to expect both pre and post-operative. * RN will educate patient on recovery time, pain management, post-operative diet, and activity restrictions. * Demonstrates the ability to communicate effectively by identifying barriers to forthcoming surgical procedure. * RN will assess pre-operative labs and monitor the blood work for any abnormalities that may prevent surgery. * RN will coordinate and facilitate the scheduling of pre-operative imaging studies (i.e. MRI, CT scan). * RN will expedite imaging study approval from the insurance company. * RN will expedite the imaging study to ensure it will get done in a timely manner. * Communicates with patients and their families about the medical plan of care and discharge plan. * Provides post-operative phone call within 24 hours of discharge. * RN will assess pain, bowel function, and the surgical site as communicated by the patient. * RN will assess how well the patient is tolerating their diet. * RN will report findings to MD. RN will work collaboratively and maintain an active communication with physicians, nursing, and other members of the multidisciplinary care team to affect timely and appropriate patient management. * Collaborates with the healthcare team in data collection and statistical activities to facilitate problem identification and resolution to improve quality of care. * Monitors documentation in the medical record to ensure patient outcomes are achieved and maintained based on established standards of care. * Participates in continuous process improvement efforts to improve patient outcomes, assures appropriate utilization of resources, and increases patient satisfaction. * Evaluates the progress of patients along the continuum and updates the MD as needed. * RN will provide Wound and Foley Care. * RN will assist MD in caring for the patient post-operative wound site with dressing changes and Vacuum Assisted Closure (VAC). * If the patient is discharged with a Foley, RN will provide discharge instructions on how to care for the Foley in regards to emptying the Foley, applying a leg bag, and removing the Foley upon post-operative visit. * Facilitates Direct Admissions. * RN will notify the Admissions Department of a direct admission. * RN will provide the necessary information pertaining to the direct admission inclusive of the patient name, demographics, labs, and imaging studies. * Assists in the coordination of two Lunch and Learn events per year. These events are community educational outreach programs generated by the Surgical Gynecologic Oncology Division. * RN will collaborate with the APN to determine topic for Lunch and Learn. b. RN will collaborate with the APN in selecting a speaker for the Lunch and Learn. * RN will provide and distribute flyers for the Lunch and Learn. * RN will coordinate the caterer for the Lunch and Learn. * RN will reserve and set up the Conference Room for the Lunch and Learn. f. RN will orchestrate the Lunch and Learn on the day of the event. * Assists in the coordination and maintenance of the Gynecologic Disease Specific Certification program. * RN will do chart reviews prior to the DSC Audit to ensure the patients are receiving the care based on the indicators chosen as noted below: * Nutrition Referrals. * Genetic Referrals. * Ongoing communication with ancillary staff in the Surgical Gynecologic Oncology Division to ensure phone calls are properly triaged, disability papers are completed, and the most recent lab and imaging studies are brought to the attention of the Clinician and are filed appropriately. * Adheres to the Medical Center's Organizational Competencies and the Standards of Behavior. Qualifications: Education, Knowledge, Skills and Abilities Required: * Registered Nurse Graduate of a NLN/AACN accredited program in nursing. * R.N. with at least 2 years clinical experience in the relevant clinical specialty. * Excellent interpersonal skills. * Strong organizational and time management skills. * Ability to work independently and exercise sound judgment in interactions with physicians, patients, and their families. Education, Knowledge, Skills and Abilities Preferred: * Oncology experience. * Surgical experience. Licenses and Certifications Required: * AHA Basic Health Care Life Support HCP Certification. * NJ State Professional Registered Nurse License.
