Position Overview: The RN Case Manager collaborates with physicians and the multidisciplinary team to develop and manage personalized care plans for an assigned caseload. **SIGN ON BONUS ELIGIBLE What You Can Expect From This Role: * Schedule: 2, 8 hour shifts per week minimum * Benefits: Full Benefits with hospital contributions * Additional Pay: Cert Pay available for qualifying certifications AND shift differentials * Bonuses: Sign on Bonus Eligible and relocation assistance available (where applicable) Qualifications: * Education: Associates' of Nursing Degree/Diploma Bachelors' of Nursing Degree * Experience: 2 years' of RN experience * Licenses/Certifications: Current RN License in Idaho For more information, please contact Julie at leguinej@slhs.org Responsibilities: The Case Manager works with physicians and multidisciplinary team members to develop a plan of care for assigned patients. * Works as clinician, consultant, educator, and researcher to facilitate collaborative management of patient care. * Works closely with physicians, nurses, social workers and a wide range of medical and non-medical professionals. * Evaluates patient progress toward outcomes by monitoring the goals, care plan and revising plan of care as necessary. * Determines need for therapeutic medical, psycho-social, and psychiatric evaluations. Reviews therapist evaluations, treatment, objectives, and plans. * Responds to patient and family needs. Serves as advocate when problems and issues arise. * Coordinates discharge and post-discharge requirements; orientates and trains family members; provides resources. * Perform other duties and responsibilities as assigned. Qualifications: * Education: Bachelors' of Nursing Degree * Experience: 2 years' of RN experience * Licenses/Certifications: Current RN License in the State of Practice
