Responsibilities:
Position Summary:
Under the direction of the Care Management Manager and in collaboration with in interdisciplinary team of Mercy Hospital staff and physicians, this position is charged with developing, implementing, and evaluating palliative care to enhance the provision of high quality palliative care services, which meet regulatory and fiscal requirements to seriously ill and dying patients in the hospital setting. Included in this responsibility is developing and maintaining strong relationships with physicians, hospital staff, community stakeholders, and system peers to deliver palliative, comfort care to seriously ill and dying patients. Specific duties may include participation in quality improvement activities; formal and informal education and support to staff, patients and their families; consultation on complex cases; appropriateness of care or level of care / utilization activities; liaison to internal and external contacts / committees / programs; compliance with regulatory / accreditation agencies, program development, facilitation and leadership. In addition, this position is responsible in working with patients and families, chaplains, hospital liaison and case management staff and physicians in assisting with decision making / problem solving and determining appropriateness for hospice services, or other end of life decisions that the patient and family may need to make.
Qualifications:
Minimum Requirements:
* CA RN license
* BLS from AHA
* 3 years of clinical experience or a comparable combination of education and experience required. Must have the ability to read, write and speak English.
* ELNEC training required within one year of hire.
