Overview:
POSITION SUMMARY: The PACE Registered Nurse (RN) Home Care Coordinator is a required member of the PACE interdisciplinary team and is expected to regularly participate in the ongoing assessment, care plan development and coordination of care of PACE participants. The principle focus of this role will be to provide, oversee, schedule, and manage the care of PACE participants in the home and community. This includes but is not limited to home care assessment, RN home care duties, scheduling, coordination, and competency management of PACE Certified Nursing Assistant caregivers.
Qualifications:
REQUIRED QUALIFICATIONS:
* Education: Bachelor's degree in nursing
* Experience: Experience as an RN in a healthcare environment. Three years' experience working with the frail or elderly population or if the individual has less than required experience, but meets all other requirements, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
* Licensure/ Certification: Registered Nurse License in Illinois; Current American Heart Association HealthCare provider BLS required prior to start date of employment. Current driver's license.
* Other Skills/ Knowledge: Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software.
* Strong analytical and problem-solving skills, with the ability to be detail oriented.
* Provides own transportation and auto insurance and abides by all laws, rules, and recommendations for safe driving while on duty.
PREFERRED QUALIFICATIONS:
* Other Skills/ Knowledge: Electronic Medical Record basic proficiency
