Position Overview
The MDS Nurse Assessor is responsible for collecting and submitting assessment data critical to the health and well-being of our residents. You will support the MDS/PPS Coordinator in completing PPS and/or OBRA assessments for both subacute and long-term care residents, as well as perform additional tasks as assigned.

Responsibilities:

RN - MDS Nurse Assessor - Essential Job Functions:

* Accurately and timely complete and submit Minimum Data Set (MDS) assessments

* Ensure compliance with Federal, State, and facility policies

* Actively participate in utilization management processes, including PDPM and state case mix systems

* Conduct resident interviews

* Compile, monitor, and audit clinical documentation

* Communicate clinical issues effectively to appropriate clinicians

* Support and assist the MDS/PPS Coordinator as needed

* Demonstrate thorough knowledge of policies and procedures

* Utilize electronic timekeeping systems as directed

* Follow facility dress code and identification policies

* Abide by all policies related to electronic devices, computers, and tablets

* Maintain compliance with company code of conduct, HIPAA, and regulatory standards

* Perform other duties as assigned by supervisor or Clinical Reimbursement Department

Qualifications:

RN - MDS Nurse Assessor - Qualifications:

* Licensed Professional Nurse (RN licensure preferred) with current state license

* Prior experience with MDS 3.0 preferred

* Long-term care experience preferred

* Excellent verbal and written communication skills

* Self-motivated, highly organized, and dedicated

* Willingness to train the right candidate
