(This differential may be adjusted or discontinued based on business requirements.) Responsibilities: * Assess patient needs, including physical, mental, financial, and spiritual aspects * Provide skilled nursing care in alignment with physician/APC care plans * Coordinate patient care through collaboration with caregivers, multidisciplinary teams, and physicians * Adjust interventions as needed based on patient responses and updated orders * Evaluate effectiveness of interventions at each visit * Develop and implement hospice care plans while ensuring compliance with regulations * Maintain productivity standards for patient visits and caseload management * Communicate visit schedules and any changes to patients in a timely manner * Guide newly hired nurses and students, serving as a mentor and resource * Ensure documentation is accurate, timely, and compliant with agency policies * Delegate appropriate care tasks to LPNs and CNAs * Participate in quality improvement initiatives and performance improvement projects * Take on leadership responsibilities as required by the department * Provide holiday, weekend, and evening coverage as needed * Perform additional functions as required for patient care and agency operations Qualifications: * Associate Degree in Nursing * Registered Nurse Licensure - Compact/Multi * Minimum one year in direct patient care setting or home health care. * Valid Driver's License * BLS required at time of hire
