				Overview:

Sign-On Bonus: $10,000

Shift: Days, Monday - Friday, for 80 hours per pay period, holiday and weekend rotation plus on-call

We are seeking a dedicated and highly skilled Registered Nurse to join our homecare team. The ideal candidate will demonstrate strong clinical proficiency, sound judgment, and a commitment to delivering patient-centered care within the home environment. This role offers the opportunity to provide individualized nursing services while supporting patient independence, safety, and overall well-being.

Responsibilities:

* Conduct comprehensive assessments and develop individualized care plans

* Deliver skilled nursing care in accordance with physician orders and organizational standards

* Monitor patient status, evaluate responses to treatment, and document all care accurately

* Educate patients and families on disease management, medications, and home safety

* Collaborate with interdisciplinary team members to ensure continuity of care

Qualifications:

Education: 

* Must have an Associates Degree in Nursing. (Must obtain BSN within 12 years of hire date)

Certification/License: 

* Active Iowa Registered Nurse licensure issued by the Iowa Board of Nursing
* Within 1 year of hire, need to obtain RN licensure in the State of Wisconsin
* Must have a valid driver's license and proof of automobile insurance coverage
