				Overview:

The Inpatient Diabetes Education RN provides individualized education to patients with diabetes during hospitalization. Primary responsibilities include assessing patient/family education needs, providing education consistent with current American Diabetes Association's National Standards of Medical Care in Diabetes, and demonstrates continued learning and knowledge of diabetes technology and devices. The Inpatient Diabetes Education RN maintains professional competency/skills and collaborates with interdisciplinary teams to provide the Best Outcome for Every Patient Every time. 

Responsibilities:

Patient Care and Education

  Conducts a comprehensive educational needs assessment through evaluation of medical and nutritional history for patients with diabetes. 

  Provides individualized 1:1 diabetes education to the patient and/or family/support persons based on the needs of the patient.

  Demonstrates knowledge in the use of blood glucose monitors, continuous glucose monitors (CGMs), insulin pumps, and home insulin supplies to provide patient/family education. 

  Reviews diabetes education consults and triages patients as appropriate based on patient conditions, needs, and discharge planning. 

  Contacts the provider as necessary for clarification of consult and/or to recommend appropriate therapy.

  Offer treatment and discharge recommendations based upon glucose patterns and current health/social variables; Assist with coordination of safe, realistic home diabetes management plan. 

  Accurately documents patient assessment, education, plan and communication with healthcare team in the electronic medical record. 

  Communicates with interdisciplinary teams to develop, communicate, and implement an individualized plan for diabetes management. 

  Monitoring of patient's plan of care, orders, and discharge plans to ensure patient safety and best outcome

Clinical Excellence & Quality Improvement 

  Evaluate and maintain inventory of diabetes related supplies within the inpatient diabetes education offices. 

  Practices according to current American Diabetes Association's National Standards of Medical Care in Diabetes. 

  Demonstrates the ability to meet the department work schedule. 

  Accountable for a safe, effective, evidence-based practice of self and team members. 

  Completes all competency/skills assessment requirements.

  Continues professional development by involvement in professional activities, continuing education &/or conferences that enhance diabetes knowledge. 

  Evaluates the diabetes education process and implements changes as indicated. 

  Participate in development / revision and review of patient education materials.

  Develops, communicates, and implements evidence-based care according to ADA Standards of Medical Care in Diabetes 

  Serves as a reliable resource and advisor to medical staff/providers/patient care staff on diabetes related issues.

  Participates in strategic planning and developing diabetes education/care related policies and procedures.

  Performs other related duties as required.

  This position may be assigned a specific mission to address in an emergency to assist in disaster response and recovery as part of the Hospital Emergency Incident Command System. Situations of a critical nature may require an individual to perform multiple tasks until additional support can be obtained.

Staff/Community Education

  Participates in educational programs for team members, other health professionals, and the public to increase knowledge and skills.

  Contributes to the education of team members, nursing and dietetic students and interns, medical students, and residents. 

  Represents the Diabetes Education Department formally and informally through interdepartmental communications and participation on UnityPoint Health-Des Moines committees, professional associations and to the public

Qualifications:

EDUCATION:
- Registered Nurse with BSN degree from an accredited college or university.

EXPERIENCE:

- 2 years' experience as a Registered Nurse.

- Preferred experience in diabetes education/diabetes self-management education.

LICENSURE/CERTIFICATION:

- Must have and maintain licensure as a Registered Nurse in the State of Iowa.

- Commitment to complete required diabetes education/CE hours within 2 years, and sit for certification exam as a Certified Diabetes Care and Education Specialist (CDCES) within 3 years of hire.

- Must maintain CDCES, once it is obtained.
