As a vital member of our Clinic RN team, you will:

* Apply your expertise in community health concepts, ambulatory clinical nursing, case management, and care coordination to positively impact patient outcomes.
* Drive accountability in facilitating communication and collaboration within the healthcare team, ensuring seamless patient/family engagement.
* Champion best practices to empower patients through engagement and self-management strategies.
* Uphold the highest standards of patient care, promoting optimal health and safety via the nursing process, guided by patient care standards and the State Nurse Practice Act.

Qualifications:

In addition to bringing your whole self to the workplace each day, qualified candidates will need the following: 

* One year experience preferred
* Nursing skills and knowledge base specific to specialty
* Graduate of an accredited School of Nursing
* BSN Preferred, encouraged to obtain within 3-years of assuming role
* Current state RN license/ Current RN license from a participating state in National Licensure Compact
* Current BLS is required
* RNs Certification in Ambulatory Nursing through the AAACN, preferred
* 1 year of Primary Care Practice experience preferred
* EPIC experience preferred

Physical Requirements-Medium Work - (Exert up to 50lbs force occasionally, and/or up to 20lbs frequently, and/or up to 10lbs constantly)
