Position Overview As a Home Health Case Manager, you will play a key role in collaborating with physicians and multidisciplinary team members to create and manage individualized care plans for a designated caseload. This is a direct care role, involving travel within Boise, ID and surrounding regions to deliver in-home care from admission to discharge. Your work will ensure that each patient receives the highest standard of care, aligned with their needs and the care plan. Qualifications * Education: ASN or BSN in Nursing * Experience: 2 years of RN experience; Home Health or Acute Care experience preferred * Licenses/Certifications: Current RN license in the state of Idaho; Basic Life Support (BLS) certification * Other Requirements: Valid driver's license and proof of auto insurance Responsibilities: The Home Health/Hospice Coordinator works with physicians and multidisciplinary team members to develop a plan of care for assigned patients. * Works as clinician, consultant, educator, and researcher to facilitate collaborative management of patient care. * Works closely with physicians, nurses, social workers and a wide range of medical and non-medical professionals. * Ensures patient is progressing towards desired outcomes by monitoring care through assessment and/or patient records. * Determines need for therapeutic medical, psycho-social, and psychiatric evaluations. Reviews therapist evaluations, treatment, objectives, and plans. * Develops patient care plans, evaluates and monitors progress, responds to patient and family needs. Serves as advocate when problems and issues arise. * Coordinates discharge and post-discharge requirements; orientates and trains family members; provides resources. * Performs therapeutic nursing interventions under the direction of a licensed physician. * Provides ongoing patient education on diagnosis, treatments, prognosis and medications. * Perform other duties and responsibilities as assigned. Qualifications: * Education: Associate's or Bachelor's of Nursing Degree * Experience: 2 years of RN experience; home health or acute care experience preferred * Licenses/Certifications: * Current RN licensure from the State of Practice * Current Basic Life Support (BLS) Certification Provider Certified through American Heart Association or American Red Cross * Valid driver's license and proof of auto insurance is required for home visits. Must use your own vehicle while in the field.
