St. Luke's Health System in McCall, ID is seeking a Home Health Case Manager to join our Home Health team. The Case Manager will occasionally be required to provide support to Hospice patients in the McCall area.
Position Overview
As a Home Health Case Manager, you will play a key role in collaborating with physicians and multidisciplinary team members to create and manage individualized care plans for a designated caseload. This is a direct care role, involving travel within the McCall and surrounding regions to deliver in-home care from admission to discharge. Your work will ensure that each patient receives the highest standard of care, aligned with their needs and the care plan.

What You Can Expect in This Role
* Full-time Schedule: Monday to Friday, 8:30 AM - 5:00 PM, with rotating evening on-call and weekend shifts
* Patient Population: Home Health and Hospice patients within the McCall service area
* Manageable Caseloads: Daily visit requirements are below the state and national average
* Compensation: Paid hourly which includes drive time and mileage reimbursement for traveling within McCall and it's surrounding regions
* Additional Pay: On-call pay and applicable differential pay

Qualifications
* Education: ASN or BSN in Nursing
* Experience: 2 years of RN experience; Home Health or Acute Care experience preferred
* Licenses/Certifications: Current RN license in the state of Idaho; Basic Life Support (BLS) certification
* Other Requirements: Valid driver's license and proof of auto insurance

Responsibilities:
The Home Health/Hospice Coordinator works with physicians and multidisciplinary team members to develop a plan of care for assigned patients.
* Works as clinician, consultant, educator, and researcher to facilitate collaborative management of patient care.
* Works closely with physicians, nurses, social workers and a wide range of medical and non-medical professionals.
* Ensures patient is progressing towards desired outcomes by monitoring care through assessment and/or patient records.
* Determines need for therapeutic medical, psycho-social, and psychiatric evaluations. Reviews therapist evaluations, treatment, objectives, and plans.
* Develops patient care plans, evaluates and monitors progress, responds to patient and family needs. Serves as advocate when problems and issues arise.
* Coordinates discharge and post-discharge requirements; orientates and trains family members; provides resources.
* Performs therapeutic nursing interventions under the direction of a licensed physician.
* Provides ongoing patient education on diagnosis, treatments, prognosis and medications.
* Perform other duties and responsibilities as assigned.
Qualifications:
* Education: Bachelor's of Nursing Degree
* Experience: 2 years of RN experience
* Licenses/Certifications: Current RN License in the State of Practice
* Valid Driver's License and Auto Insurance if work related travel is required for position
* Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross.
