The Program Director for the Navigation/Diversion and Home at Last program works with young adults, eligibility at entry ages 18-24, who meet HUD categories 1, 2, or 4 for the definition of homelessness to create sustainable housing and supports. A Y Social Impact Center Housing and/or Peer Navigator connect youth to housing resources and help them to navigate the complexities of the homelessness crisis response system. Participants can be provided with move-in assistance and up to four months of rent assistance based on their needs. The Director also oversees the Home at Last project staff providing services to Young Adults in a permanent supportive housing project conducting case management including life skills and connecting the participants to long term, adult systems for ongoing support and access to permanent housing vouchers.
