Responsibilities:
* Deliver comprehensive primary care, including preventive, acute, and chronic disease management.
* Collaborate with care teams to ensure high-quality, patient-centered care.
* Maintain accurate documentation in the EMR system.
* Support health education, wellness initiatives, and quality improvement efforts.
Qualifications:
* Board Certified or Board Eligible in Family Medicine or Internal Medicine
* Eligible for or active South Carolina Medical License
* Passion for high-quality, patient-centered care
