Overview: Diagnose, treat patients, and supervise medical residents in outpatient at the hospital and in ambulatory health care center in Brooklyn. Responsibilities: Is responsible for all aspects of patient care and appropriate supervision of medical residents who see all patients first and under guidance of the attending. The resident shall under the supervision of the teaching attending be responsible for the following: Depending on the PGY level, diagnose and manage all acute and chronic medical issues with sub-specialty and specialty input where indicated. * Perform comprehensive health assessment for all patients. * Complete a history, and physical examination for all patients. * Elicit patient medical and social history, perform a review of systems, initiate lab and radiological tests and studies, provide health teaching and counseling, monitor and manage chronic conditions, and prescribe care and treatment according to the policies and procedures of Interfaith Medical Center and all medical standards and guidelines provided by regulatory agencies e.g., Joint Commission, NCQA, New York State Department of Health, and all other regulatory bodies. * Consult with other members of the patient care team regarding the individual needs of patients, provide support for recommended actions and refer patients to appropriate clinic and/or community resources. * Advise patients concerning diet, hygiene, and methods for prevention of disease. * Refer patients to medical sub-specialist, other practitioner(s) for specialized treatment and/or refer patients to educational and support groups. * Ensure that all communications with patients are culturally and linguistically appropriate and patient education and care are delivered according to the cultural and linguistic needs of the patient. * Perform medication reconciliation for each patient and work with other members of the health care team to ensure that when changes are made to the patients' medication regimen the patient is provided with education regarding the purpose, administration of the medication and side effects or other problems to report to their care provider. The attending shall: * Attend, and actively participate in departmental meetings, PCMH team meetings, and huddles. * Order the vaccination of patients to immunize patients from communicable diseases. * Participate in case management meetings, and all staff meetings as appropriate. * Ensure that all communications with patients are culturally and linguistically appropriate and procedures, patient education and care are delivered according to the cultural, and linguistic needs of the patient. * Participate in quality management and performance improvement for care evaluation and research activities. * Meet all quality standards of care and measures including but not limited to, preventive care and screenings (Breast Cancer, Colorectal Cancer, BMI, Urinary Incontinence, Osteoporosis, pain assessment and follow-up, screening for clinical depression and follow-up plan, screening for alcohol use, tobacco use and prescribes cessation intervention, high blood pressure, fall risk, abuse and neglect, administer appropriate vaccinations and immunizations for adults, children and adolescents, provide comprehensive care and management of diabetic patients, age-appropriate screenings for HIV/AIDS and sexually transmitted diseases such as Chlamydia, Gonorrhea, Syphilis, Tuberculosis Prevention for Psoriasis, Psoriatic Arthritis and Rheumatoid Arthritis Patients on Biological Immune Response Modifier, and screen for other infectious diseases such as Hepatitis C Virus. * Assist patients with chronic conditions to navigate and access chronic care services, in conjunction with the patient care team including quality care, health education and social services. * Responsible to give monthly evaluation of residents. * Responsible for ambulatory lecture series in the morning. * The physician is expected to participate in QA projects including core measures. Qualifications: Education & Certifications: * Board Certified in Internal Medicine or completion of Internal Medicine Residency. * Must qualify for New York State medical license. * Current I-STOP and e-prescription registration. * Current Board certification, or board certification by November of the current year. Knowledge and Skills: * Ability to communicate effectively in English, both verbally and in writing. * Good interpersonal skills a must. * Good written and verbal communication skills. * Basic knowledge of computers and word processing skills. Physical Requirements: * Involves standing, walking, sitting, lifting and talking. * May include some repetitive motions.
