Overview: PHYSICAL THERAPIST - HOME HEALTH (PRN) COVERAGE AREA - MONTROSS, VA OFFICE LOCATION: IRVINGTON, VA Our Home Health Physical Therapists (PT) are the heart of our organization! Our comprehensive benefits include: * Competitive pay + mileage reimbursement * Paid time off * 401(k) program * Flexible Spending Account (FSA) * Employee Assistance Program (EAP) * Vendor discounts Responsibilities: * Provide physical therapy services to patients according to a written physician's plan of care * Initiate physical therapy program and instruct other personnel and/or family/caregiver members as appropriate * Prepare and submit clinical and progress summaries * Provide in-service education programs for nursing organization personnel as needed * Participate in discharge planning * Supervise Physical Therapy Assistants according to policy and state regulations * Ensure compliance with all state and federal legal and regulatory requirements * Additional responsibilities as assigned Qualifications: * Graduate of a physical therapist education program approved by CAPTE with current licensure to practice in state of operation * 2 years of experience in adult physical therapy * Home health experience preferred * Commitment to clinical and documentation excellence
