Responsibilities:

Position Summary: 

Participates as a member of the health care team, working with professional and non-professional staff and associated professionals in providing care which provides optimum home care and continuity of care to referred patients, addressing the age of the patients served.

Qualifications:

**Offering a 7,500 Sign-on Bonus for qualified applicants

Minimum Qualifications:

* Current CA Physical Therapy licensure
* BLS CPR certification (Must be through the American Heart Association)
* 1 year experience as a physical therapist
* Knowledge and ability to perform current physical therapy techniques
* Knowledge of physical therapy standards procedures and principles
* Ability to operate physical therapy equipment for patient care
* Ability to implement practical application of knowledge
* Ability to work independently and autonomously
* Ability to teach and instruct patients caregivers and staff
*  

Preferred Qualifications:

* 6 months to one year home health experience
