Responsibilities:

Job Summary: 

In keeping with the Hospice/Home Healthcare philosophy, the Physical Therapist performs all phases of physical therapy evaluation, treatment planning, treatment and documentation, and performs other tasks related to quality of care and effective department function.  Performs such typical distinguishing duties as:  evaluating patients, participating in the treatment program planning; administering physical therapy modalities, procedures and tests; documenting accurate patient records and progress notes; maintaining appropriate oral or written contact with the referring physician and co-workers.  Ensures a clean and safe work environment by being alert for unsafe conditions and malfunctioning equipment and using appropriate procedures for their correction; supervises Certified Home Health Aides in the participation in the overall treatment program.  Assists DPCS, Hospice Supervisor, and QI Coordinator with QI as required.  Performs other related duties, such as participating in all departmental functions and activities as required, including in-service education, clerical and other tasks relevant to effective department function as necessary.

Job Duties:

* Performs patient initial and reevaluations; establishes and implements treatment plan, accordingly.

* Administers treatment plan using accepted therapy procedures; oversees therapist extenders (aides) in the implementation of established treatment plans.

* Teaches patient and/or caregiver in functional activities and treatment techniques of physical therapy as appropriate to meet their needs.

* Documents daily on the appropriate forms using clear and concise data. Records treatments accurately and communicates patients status/progress in Patient Care Conference and DIC summaries. Submits documentation at least weekly.

* Communicates regularly with other members of the medical team regarding goals, progress, and rehabilitative potential. Works as a collaborative member of the treatment team.

* Maintains the standards of conduct as promulgated by the American Physical Therapy Association (as a registered Physical Therapist); adheres to the professional code of ethics in the performance of duties.

* Demonstrates specialized clinical knowledge and skills in an identified area of expertise.

* Complies with safety, infection control, Dignity Health Integrity Program, and other organizational policies (90-day evals- department specific orientation checklists completed; annual evals -annual competency checklists completed).

* Complies with the organization's dress code.

* Arrives at work on time, as defined by the Attendance and Timeliness Policy.

* Attends work consistently, as defined by the Attendance and Timeliness Policy.

* Actively supports the organization's mission, philosophy and strategic plan.

* Demonstrates good time management and priority setting.

Qualifications:

Experience Required: 

Hospital Based:Minimum of 3 years clinical experience.

Evidence of ability to function with a high degree of independence and competence.

Manages audits feedback on evals and precepts. 

Home care based:3 - 9 years of experience.

Education Required

Graduate of an accredited physical therapy program

Licensure Required

Current California license in Physical Therapy

Current CPR

Must have current CA Driver license

Clean Driving record with proof of liability insurance for automobile
