Overview: Dignity Health Mercy San Juan Medical Center is a 384-bed not-for-profit Level 2 Trauma Center located in Carmichael California. One California Responsibilities: The incumbent demonstrates competency in patient care for neonate, pediatric, adult, and geriatric patients. May be required to direct the work of a Physical Therapy Assistant. * Meets the discipline-specific productivity standard. * Demonstrates competency in the care of patients during field observation by the clinical supervisor. * Completes and submits documentation following established guidelines as measured by a designated evaluation tool. * Physician orders are present and written correctly. * Documents follow up of identified problems. * Reviews and documents clinical progress toward goals. * Demonstrates coordination of care by appropriate communication with other disciplines and team members, and documents that communication as measured by a designated evaluation tool. * Meets agency/regulatory requirements as measured by a designated evaluation tool. * Documents appropriate management of visit utilization and authorization status as measured by a designated evaluation tool. * Meets regulatory requirements for supervisory visits as measured by a designated evaluation tool. Qualifications: Required * Completed an accredited physical Therapy degree program and be board eligible and able to obtain CA PT license within 6 months or have already obtained a CA PT License and Previous experience as a PhysicalTherapist or Physical Therapist student/ intern, upon hire * Physical Therapist: CA, within 6 - months and * Basic Life Support - CPR, upon hire and * Driver's License: CA, upon hire and * Auto Insurance, upon hire
