Ottawa Home Health serves over 400 patients across multiple counties along the I-80 corridor. We have an extensive team of experienced clinicians to meet the needs of our patients in the Illinois Valley. POSITION SUMMARY: The Physical Therapist assists patients in the restoration of physical function through evaluation and treatment.

Qualifications:

REQUIRED QUALIFICATIONS

Education: 

* Doctoral degree in Physical Therapy 

*Those licensed after 2016 must have a Doctoral degree. Those who were licensed prior to 2016 are exempt from this requirement as grandfathered under prior IDFPR or MBPT requirement for licensure (which allowed bachelor's/master's degree; the normal industry requirement prior to 2016).

Licensure/Certifications: 

* Respective State license in Physical Therapy 
* American Heart BLS 
* Valid state Driver's license and Insurance 

Other skills/knowledge: 

* Demonstrated knowledge and skills necessary to provide patient care appropriate to the population served. 
* Strong interpersonal, written and oral communication skills to communicate plan of care and educate family members on care. Communicates effectively with other treatment team members when gathering and exchanging patient condition information. 
* Analytical ability to evaluate patients' physical and functional status, establish therapy goals, formulate treatment plans consistent with goals, observe patients and document status, and assist techs and assistants in resolving problems.  
* Excellent computer skills and the ability to process detailed information in an accurate manner. 

PREFERRED QUALIFICATIONS

* One year experience in hospital or Home Care setting
