Physical Therapist - Clinical Coordinator

Responsibilities:

* Perform patient evaluations and develop personalized treatment plans.
* Regularly reassess treatment effectiveness and participate in staffing meetings and family conferences.
* Conduct discharge evaluations, recommend home and community safety modifications, and provide education and referrals.
* Communicate patient progress and needs to the treatment team, physicians, patients, and families.
* Collaborate with other disciplines to coordinate care and enhance patient outcomes.

Qualifications:

* Degree in Physical Therapy from an accredited program.
* Active Physical Therapist license in the practicing state.
* CPR or Basic Life Support certification per state regulations.
* At least one year of physical therapy experience preferred.
* Knowledge of rehabilitation techniques for complex neurological injuries.
* Strong verbal and written communication skills.
* Ability to lift 50 pounds; safely move/transfer patients up to 100 pounds.
* Physical stamina: fine motor skills, visual acuity, extended standing/walking, pushing, pulling, bending, stooping, reaching, kneeling, and positioning.
* Health screenings or exams may be required per location or state policies.
