Pharmacy Technician Medication Reconciliation II

				Responsibilities:

* Accurately validates prior-to-admission medication lists and takes medication & allergy history from patients.
* Contacts outpatient entities as needed for additional patient medication information.
* Communicates information to the health care team.
* Documents/corrects information in patient medication list in EPIC.
* Accurately and timely prepares all medications, including IV medications,
* under the direct supervision of the pharmacist as per policy and regulation.  
* Completes expired medication inspections
* Answers phones to assist in dispensing questions
* Functionally uses the hospital information systems
