Overview: Under the general supervision of the Director of Pharmacy and/or the Pharmacy Supervisor, and according to Federal and State laws compounds and dispenses medications and pharmaceutical supplies upon receiving written prescriptions from physician. Responsibilities: * Under general supervision and according to hospital policies and the laws of New York State: compounds and dispenses medications and supplies to patients and other professionals. * Interprets and evaluates physician's orders to prepare, calculate and dispense appropriate medication and medication dosage forms. * Communicates with prescribes and nursing personnel according to intervention procedure to clarify or change orders that are unclear, illegible, incomplete or questionable. * Recommends changes in drug therapy when needed. * Documents intervention activities. * Insures resolution of identified problems. * Accurately prepares labels and dispenses medications to all areas of the hospital in accordance with all policies, laws and regulations. * Accurately enter medication orders into the computer. * Reviews, maintains and screens medication profiles for allergies, food/drug interactions, drug/drug; interactions, and therapeutic duplications. * Provides information on the contents, therapeutic actions, interactions and possible adverse reactions of medication to the physicians, nurses and patients of the hospital. * Compounds and labels I.V. admixtures, chemotherapies and parenteral nutrition solutions according to established procedures. * Supervises and checks the work of the Technicians and Aide in both sterile and non-sterile medication work areas. * Dispenses controlled substances in accordance with al applicable regulations. * Assists in the orientation and training of new personnel. * Participates in educational programs both inside and outside the department as required by law. * Performs and documents monthly medication area inspections. * Works in all service areas of the department. * Demonstrate knowledge and competence in the consideration of patient age as a factor in dose calculation for the neonate, pediatric, adult and geriatric patient. * Performs other assigned duties as may be necessary for the quality assurance and performance improvement programs of the department. * Attends and participates in committee meetings and functions requires by the hospital.. * Performs other duties as may be needed to assure the maintenance of optimal patient care services. Qualifications: Education: * Bachelor's degree in Pharmacy, required. Experience: * Minimum one (1) year hospital pharmacy experience. Knowledge and Skills: * Ability to communication in English, both verbally and in writing. * Knowledge of computers and word processing skills. Licenses, Certifications: * New York State Pharmacy license and current registration. Physical Requirements: * Involves standing, walking, sitting, talking, hearing and bending. * May include some repetitive motions. * Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the position without compromising patient care.
