				Overview:

Patient Placement Specialist - Transfer Center

Available Shifts: 0.3FTE, 7p-7a (1-12 hour shift)

Onsite: Des Moines, IA

The Patient Placement Specialist will organize and prioritize bed placement for their assigned pod in partnership with the transfer center hospitalists, nurses, and market points of contact. The Patient Placement Specialists serves hospital community by adhering to professional standards, hospital policies and procedures, federal, state and local requirements and accrediting bodies.

Responsibilities:

* Assigned to a pod of hospitals to place patients for that shift
* Coordinates patient placement assignments and re-assignments throughout all hospital units; verifying bed requests to admission orders for placement using various data sources.
* Communicates with intake/lead nurses, hospitalists, unit leaders and charge nurses, and nursing supervisors to place patients, resolve bed shortage conflicts and convey the status of available beds.
* Assist in placement of planned admissions in partnership with unit leaders.
* Establishes effective working relationships with cross-functional team(s).
* Responds timely, effectively and appropriately to deliverables
* Identifies and responds appropriately to both internal and external customer needs utilizing available resources
* Partners with the Clinical Capacity Expeditor to identify and resolve placement or discharge delays.

Qualifications:

Education: 

* High School Diploma required

Experience: 

* 2 years of Clinical Healthcare experience required

Licenses/Certifications:

* BLS (can obtain with 90 days of hire)

Knowledge/Skills/Abilities:

* Excellent written and verbal communication
* Highly organized
* Proficient with technology including Microsoft products and EPIC
* Able prioritize effectively when faced with multiple tasks simultaneously
* Excellent customer service skills; able to navigate complex or challenging discussions
