Overview: As a member of the interdisciplinary team, the PCT gives direct care to patients, under the supervision of the nurse. The PCT is responsible for a variety of tasks and services to assist in the provision of safe care, and comfort for patients. CNA not required. Responsibilities: Clinical Excellence/Patient Care Assists patients with tasks they are unable to do for themselves, (ADL's, feeding, grooming, mobility, etc). Monitors the health of patients via vital signs, glucose assessments, safety rounds, and reports as appropriate to the nurse. Observes patient's condition, and any changes in their physical, emotional or mental status, and communicates the status of the patient, including any changes, periodically, to a supervisor, case manager, or nurse. Promptly completes documentation to assure an accurate legal record of patient's care. Follows infection prevention measures. Environment Assists the team to ensure adequate stock and supply is available for patient care. Supports the team in keeping patient rooms and the work area safe and clean. Clerical Assists the team to respond to patient calls in a timely fashion. Supports the medical team in handling paperwork. Basic UPH Performance Criteria Demonstrates the UnityPoint Health Values and Standards of Behaviors as well as adheres to policies and procedures and safety guidelines. Demonstrates ability to meet business needs of department with regular, reliable attendance. Employee maintains current licenses and/or certifications required for the position. Practices and reflects knowledge of HIPAA, DIA, OSHA and other federal/state regulatory agencies guiding healthcare. Completes all annual education and competency requirements within the calendar year. Participates in department/hospital meetings, programs, performance improvement, etc. Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Qualifications: * BCLS Required withing 180 days of emloyment * Writes, reads, comprehends and speaks fluent English * Microsoft Office - Basic computer skills * use of usual and customary equipment to perform essential functions of the position
