Overview: The Patient Care Manager Assistant supports the Clinical Manager in coordinating staff to ensure continuity and quality of care for assigned home health patients. Ensures team members are informed, skilled, and deliver care efficiently in compliance with physician orders and regulatory guidelines. Responsibilities: * Respond to phone inquiries from field staff, physician offices, referral sources, and patients; take messages and orders as needed. * Monitor incoming faxes and relay information to appropriate personnel. * Coordinate communication and transmit relevant documents to physician offices. * Track pending lab results, facilitate follow-up, and communicate findings to physicians and relevant personnel. * Monitor hospitalized patients to support effective care coordination. * Support Clinical Manager in managing software system reports to enhance care coordination and utilization management (e.g., therapy utilization, missed visits, documentation gaps). * Assist with payer source authorization processes as directed. * Conduct follow-up actions based on health check calls or tele-monitoring alerts; ensure communication with team members. * Compile patient lists and prepare documentation for weekly case conferences. * Deliver direct patient care within scope of practice and per physician orders; coordinate with healthcare team; do not modify plan of care. * Complete required learning modules, assignments from Branch Director and state-specific training as mandated. * Support documentation audits to ensure completeness and accuracy of clinician submissions. * Participate in and comply with the agency's episode management protocols. Qualifications: * Completion of an accredited Licensed Practical Nurse (LPN) program * Active LPN license in state of practice * Prior experience in home health or clinical care setting preferred * Current CPR certification * Valid LPN license in state of practice * Valid driver's license, vehicle insurance, and reliable transportation * Compliance with any additional state-specific requirements
