Role:
RN Flex Patient Care Coordinator coordinates the care for Hospice patients who reside in the Hospice House facility. Coordination of care includes: coordination and direction of the interdisciplinary team, patient, family and facility staff; patient symptom management; and assessment of patient and family physical, psychosocial, cultural and spiritual needs. The incumbent provides end-of-life education to patient, family and facility staff and advocates for patients and families to ensure patient-focused care. RN Flex Patient Care Coordinator exercises independent judgment consistent with professional licensure and applies skills obtained through academic and clinical preparation. Unusual problems or proposed deviations from guidelines, practices or precedents are discussed with a manager or physician before being initiated. RN Flex Patient Care Coordinator assigned to provide coverage as assigned for nursing staff on vacation, call outs or to assist with extra patient loads.
Qualifications:
Licensure/Certification/Education/Requirement::
Graduation from an accredited school of nursing, and two years of related nursing work experience in an acute clinical care setting that would demonstrate attainment of the requisite job knowledge skills/abilities. Work experience in one of the following clinical specialty areas is required: Oncology, Medical Surgical, Nutrition Support, Pediatrics, IV Therapy, Gerontology, and Home Health. BSN degree is preferred.
Current licensure as an RN by the Washington State Board of Nursing.
Current Healthcare Provider BLS Certification.
Current Department of Health Food Handler's permit issued by the State of Washington.
