Practice Highlights: * Practice consists of an average of six home visits per day * Call is taken in rotation with whole team, one week at a time. * Some hospice care will be a part of this role Responsibilities: Summary: Serves as a consultant in the field of Palliative and Hospice Medicine, in an outpatient setting. Responsibilities: * Collaborates with colleagues in the management of patients, in outpatient settings * Offers care to patients with critical and life-limiting illness, focused on assisting them to achieve medical treatment concordant with their goals. * Offers expert symptom management to assist patients in achieving maximal quality of life, despite their illness. * Builds effective relationships with referring physicians and care teams, strives to improve collaboration centered on the care of these patients. * In both Palliative and Hospice settings, works with an interdisciplinary team, offering support to both patients and families. * Works with colleagues to improve the quality of care offered patients Other information: Nurse Practitioner - Masters degree in Nursing, New Mexico Nurse Practitioner license, two (2) years working experience as a nurse practitioner. Physician Assistant - Certified Physician Assistant, two (2) years working experience as a Certified Physician Assistant, and licensed by the New Mexico Board of Medical Examiners required.
