Overview: UnityPoint Clinics Clinic RN, LPN or CMA Northridge Family Medicine - Hiawatha PRN As Needed Assist provider and other clinical staff with obtaining basic vital information preparing patient for examination, treatment and minor surgical procedures. Responsible for tracking system for follow-up appointments related to office visits, laboratory tests and procedures to regularly follow patients monitoring disease status. Schedule referrals for patients with specialist or outpatient as requested by the provider. Complete appropriate forms or requisitions needed for referrals or lab testing, including appropriate CPT/ICD-10 coding. Answer questions and provide teaching to patients and family members. Responsibilities: * Assist in the delivery of primary health care services and patient care management. * Demonstrates competence in the skills necessary to carry out assigned duties. * Administer medications by all routes. * Apply Nursing Process (Assessment, Diagnosis, Planning, Intervention and Evaluation - within licensure guidelines) * Act as a liaison with patients, family and providers consistently demonstrating smooth and efficient telephone communication when covering provider call and scheduling patient appointments. * Obtain complete history regarding health problems according to protocols. * Document accurate information regarding health concerns and/or condition from patient or family member. * Telephone Triage by providing and documenting advice for home care of illness/condition; conducting follow-up phone calls regarding patient's health and progress as necessary. * Ability to assess urgency of condition for immediate attention regarding illness related calls and assign priority. * Perform other duties as requested by Clinic Administrator to facilitate the smooth and effective operations of the office Qualifications: * Graduate of an accredited program for Registered Nurses. * Current license to practice nursing in the State of Iowa/Illinois for Registered Nurse. * Mandatory Reporter certificationCPR certification. * Meets educational/competency requirements per policy. * Strong interpersonal skills. * Ability to work as a team member. * Ability to understand and apply guidelines, policies and procedures. * Previous clinical experience in a medical office preferred. * Strong computer skills. * Strong verbal and written communication skills
