What you can expect as an Occupational Therapist- Rehabilitation Home Care Coordinator:
* Schedule: Monday-Friday 8:30AM-5PM
* Manageable case-loads (Daily visit requirements below state and national average)
* Paid for drive time
* Mileage reimbursement (Geographical Area: Meridian team serves the following counties in Idaho: Ada, Boise, Canyon, Elmore, Gem, Owyhee, Payette, and Washington counties)
Qualifications:
* Education:
* Degree from an accredited program.
* Experience:
* 2 years relevant experience
* Licenses/Certifications:
* Valid license to practice occupational therapy in state of practice.
* Valid Driver's License and Auto Insurance.
* Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross
* Preferred Qualifications:
* Home Health experience
Responsibilities:
The Rehabilitation Home Health Coordinator works with physicians and multidisciplinary team members to develop a plan of care for assigned patients.
* Works as clinician, consultant, educator, and researcher to facilitate collaborative management of patient care.
* Works closely with physicians, nurses, therapists, social workers and a wide range of medical and non-medical professionals.
* Ensures patient is progressing towards desired outcomes by monitoring care through assessment and/or patient records.
* Determines need for therapeutic medical, psycho-social, and psychiatric evaluations. Reviews therapist evaluations, treatment, objectives, and plans.
* Develops patient care plans, evaluates and monitors progress, responds to patient and family needs. Serves as advocate when problems and issues arise.
* Coordinates discharge and post-discharge requirements; orientates and trains family members; provides resources.
* Provides ongoing patient education on diagnosis, treatments, prognosis and medications.
* Administers interventions consistent with treatment plan based on evidence based best practice, provider protocol and governing standards.
* Perform other duties and responsibilities as assigned.
Qualifications:
* Education: Degree from an accredited program.
* Experience: 2 years relevant experience
* Licenses/Certifications: Valid license to practice physical, occupational or speech therapy in state of practice. Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross. Valid Driver's License and Auto Insurance.
