Responsibilities:
* Provide comprehensive primary care, including preventive care, chronic disease management, and acute visits.
* Work closely with physicians and care teams to deliver coordinated, high-quality care.
* Maintain accurate and timely documentation in the electronic medical record (EMR).
* Promote patient education, health maintenance, and wellness initiatives.
Qualifications:
* Graduate of an accredited Nurse Practitioner program (FNP preferred)
* Board Certified as a Nurse Practitioner (AANP or ANCC)
* Active or eligible South Carolina NP license and DEA license
* Experience in primary care or internal medicine preferred
* Excellent communication and collaboration skills with a patient-centered approach
