Responsibilities: This is a PRN position As a Nurse Practitioner with CommonSpirit Health at Home, you'll provide face-to-face assessment services to patients to assist the Medical Director and team to determine each patient's continuing eligibility for palliative care. Responsibilities: * Provides care following the plan of treatment and patient care plan to assist in determination of palliative care eligibility. * Establishes a plan of care for continued needs based on the client assessment. * Reports changes in patient condition to appropriate personnel and the attending physician. * Provides patient care education, counseling, medication advice, instructions and follow-up to clients and other caregivers. * Participates in coordination of services with other disciplines and community resources as needed. * Evaluates and revises care plans/assessment sheets based on changes in patients and/or the environment. * Assists with the development of educational programs in the palliative care program for diverse age groups. * All other duties as assigned. Qualifications: * Graduate of an accredited school of nursing. * Currently licensed to practice in the state serviced, with Masters in nursing. * At least 1 year of nursing experience * Must obtain and maintain current CPR * Strong critical thinking, clinical expertise, case management, and a passion for patient care. * Valid driver's license and auto insurance coverage according to the CHI Health at Home policy.
